Exercise Referral
What is Exercise Referral?
Exercise Referral is 12 week programme of personalised physical activity support for people who have a stable, mild to moderate long term health condition.
The Exercise Referral Scheme is available to people aged 16 years and over with an existing health condition that could be improved by physical activity and aims to get participants who are not currently active feeling more confident with exercise. The programme will be adapted by our experienced instructors to best support the management of your health condition taking into account your goals.
You'll need to ask your GP to be referred. Full eligibility details can be found in the document below. If you are eligible you'll be invited to a welcome meeting. This will typically take place within a local leisure facility on a 1-1 basis with an exercise referral instructor. You'll be invited to discuss your goals and activity preferences to help create your own tailored 12 week programme.
There are a range of available activities across different areas and venues:
- Fitness/gym based personalised exercise programmes
- Swimming pool-based activities including aqua aerobics
- Low impact group exercise classes such as Tai Chi or seated exercises
Regular reviews will take place to adapt your programme if required throughout the 12 weeks. At the end of the programme we will review your progress and look at ways you can continue being active.
